WELCOME AND THANK YOU FOR CHOOSING OCEAN PHYSICAL THERAPYBelow, please find the Women’s Health Eval Consent and Welcome Packet. New Patient forms are coming soon. Please arrive 30 minutes early (15 minutes if your paperwork is completed) for the first appointment and allow approximately 1 – 1 1/2 hours for your first visit. It is recommended that you schedule out your appointments for the duration of the prescription as indicated by your doctor.

Please bring your insurance cards, driver’s license, prescription and any X-Ray/MRI reports or surgery reports. Wear comfortable clothing that will allow access of the body part to be treated. We are located in the Pacific Pointe business park at the corner of Calle Amanecer and Calle Negocio at 901 Calle Amanecer, 3rd Floor, Suite #320 in San Clemente; feel free to call for directions if needed.
